As part of our needle and syringe programme contract, we have to keep track of items given out. We won't log any personal info here, but we do ask you to log the numbers of items you are giving each person along with any related advice. 1 interaction = 1 form completed, please do not log all of the Hub shift's interactions in one go here.

Our NSP contract requires us to collect some demographics information. Let the person know this is voluntary and that they can answer "Prefer not to say" for any/all. You do not have to ask all of these either, and if you know the answers to some of these just naturally from the conversation, that is even better! Do not force a response to any of these at the expense of the quality of the interaction itself.
