Please make sure one person completes this after every shift! You should not leave the debrief without this being confirmed as SUBMITTED (it should be done onsite and everyone on shift consulted to make sure no notes are missed).

For any supplies given out on shift, please use the NSP tracker per interaction instead of this form.

  • Please make notes of any particular drugs discussed or any local drug trends including any recent overdoses mentioned, new additions to menus, changes in price, and so forth
  • Was the shop itself busy, and if so, did you take the chance to cross promote the hub? If so, how did that go?
  • If outreach occurred, write down where was visited and what it was like
  • If you used the whiteboard or other items to 'gameify' hub please record that as well! For instance, making a true or false board to engage passers-by
